Stop Work Authorization & Assumption of Responsibility

By executing this Stop Work Order, the undersigned (“Authorizing Party”) affirms that they are duly authorized by the requesting party, property owner, or responsible entity to suspend all response, mitigation, and cleanup activities performed by Enviro Medics LLC (“Contractor”).

Upon execution of this Stop Work Order:

  1. Immediate Cessation of Services
    Contractor shall immediately cease all work, secure equipment as reasonably practicable, demobilize personnel, and depart the site. No further mitigation, containment, monitoring, or response activities will be performed after the time this form is executed.

  2. Material Change to Scope & Transfer of Control
    Execution of this Stop Work Order constitutes a material change to the scope of services and a voluntary assumption of control of the incident by the requesting party.

  3. Transfer of Responsibility
    From the moment this Stop Work Order is executed, all responsibility for the incident, including but not limited to:

    • Spill migration or expansion

    • Release of liquids, vapors, gases, or solids

    • Environmental contamination

    • Property damage

    • Personal injury or exposure

    • Regulatory consequences

    is fully assumed by the Authorizing Party and/or the requesting party.

  4. Assumption of Risk & Limitation of Liability
    The Authorizing Party acknowledges that stopping work may result in worsening conditions, increased hazards, regulatory exposure, or additional damages. Contractor shall have no responsibility or liability for any conditions, damages, injuries, losses, or consequences arising after the execution of this Stop Work Order.

  5. Authority Representation
    The undersigned represents and warrants that they have full authority to issue this Stop Work Order. If the undersigned lacks such authority, they personally assume full responsibility and liability for all consequences resulting from the cessation of work.

  6. Materials and Site Conditions
    Following execution of this Stop Work Order, the Authorizing Party assumes responsibility for all materials, equipment, waste, absorbents, containers, product, leaks, vapors, gases, or hazardous conditions present at the site at the time work is stopped.

  7. Billing and Demobilization Costs
    The requesting party agrees to pay Contractor for all costs incurred up to the time of work stoppage, including but not limited to:

    • Labor performed

    • Equipment usage

    • Materials and consumables used or deployed

    • Mobilization and demobilization time

    • Administrative and documentation costs

    All charges will be billed in accordance with Contractor’s standard rates and terms.

STOPPING WORK MAY RESULT IN WORSENING CONDITIONS, INCREASED CONTAMINATION, PROPERTY DAMAGE, PERSONAL INJURY, REGULATORY ACTION, OR OTHER SERIOUS CONSEQUENCES.

By signing this form, you are directing Enviro Medics LLC to immediately stop work and are assuming responsibility for the incident from that moment forward.
